Heart & Lung Physio Assessment and Rehabilitation Process

Prior to scheduling a home visit with you, relevant information will be obtained from both you and your primary doctor such as:

  • Reason for referral
  • Medical history
  • Any ongoing medications
  • Any precautions and contraindications
  • Premorbid status

After obtaining the relevant information, an initial assessment will be conducted by our physiotherapist to evaluate your baseline function, before tailoring an individualised home therapy programme for you.

Frequently Asked Questions

Can I be seen at home for the supervised exercise programme?

Yes, we will be able to come to your home to guide you in the suitable exercises you can do according to your home set-up. However, it is recommended that you come to the clinic for the first and last session for us to perform the appropriate assessments to monitor your progress.

My loved one requires chest physiotherapy services, is there a home therapy visit for this?

Yes, our therapist will be able to travel to your home to conduct chest physiotherapy for your loved ones. They will be utilising your home suctioning machine and/or your own cough assist machines and/or other respiratory adjuncts on your loved ones during the chest physiotherapy sessions.

How many sessions will there be for home therapy?

The number of sessions will be dependent on our therapist’s assessment. As our sessions are individually tailored for each and every client, the number of sessions may differ from person to person. Reassessment of your functional status and re-establishment of your goals will be done routinely and timely to ensure that you are progressing optimally.

My loved one has been discharged home with a home ventilator machine, can I sign up for this service?

Yes, you can!

Our therapists have experience in managing ventilated patients, and will be able to assist in chest physiotherapy for your loved ones safely and effectively.
